Women's Bone Health & Osteoporosis Prevention: The Complete Guide for Every Indian Woman — By Dr. Chetna Jain, Gurgaon

Here is something that surprises most women when I tell them: osteoporosis — the condition that causes bones to become thin, weak, and dangerously prone to fracture — is not something that simply happens to older women out of nowhere. It is a condition that begins developing silently, often as early as a woman's 30s, and accelerates dramatically after menopause. By the time a fracture happens — a wrist, a hip, a vertebra — significant bone loss has already been occurring for years, sometimes decades, without a single symptom.

I am Dr. Chetna Jain, a gynaecologist in Gurgaon, and bone health is one of the topics I feel most passionately about — precisely because it is one of the most preventable health crises facing Indian women today, and one of the least discussed. So today, let us have a proper conversation about your bones — what they need, what threatens them, and what you can do right now, at whatever age you are reading this, to protect them for the decades ahead.

Why Indian Women Are at Particularly High Risk of Osteoporosis

Osteoporosis is often called a Western disease, but that assumption could not be more wrong when it comes to India. Studies estimate that over 50 million Indians are affected by osteoporosis or low bone mass — and women account for the overwhelming majority. Indian women face a uniquely elevated risk for several converging reasons.

First, Vitamin D deficiency is near-universal in urban India — a paradox in one of the sunniest countries in the world, driven by indoor lifestyles, sun avoidance, air pollution, and darker skin pigmentation that requires more sun exposure to synthesise adequate Vitamin D. Vitamin D is essential for calcium absorption, so without it, the calcium you consume cannot be properly used by your bones. Second, Indian diets — particularly in vegetarian households — are frequently low in readily absorbable calcium. Third, the average age of menopause in India is around 46 years — approximately five years earlier than the global average. This earlier menopause means Indian women spend more years of their lives in the low-oestrogen, high-bone-loss postmenopausal state. The result is that many Indian women are entering menopause with bone density that is already lower than ideal — and then experiencing sharp postmenopausal bone loss on top of that.

Understanding Bone Loss: How It Actually Happens

Your bones are not the static, unchanging structures they might seem. They are living tissue, continuously being broken down by cells called osteoclasts and rebuilt by cells called osteoblasts. Peak bone mass — the maximum density your bones will ever achieve — is reached somewhere between your late 20s and early 30s. After that, bone formation gradually begins to fall behind breakdown — and from the mid-30s onwards, net bone loss begins.

This process accelerates dramatically at menopause. In the first five to seven years after menopause, women can lose up to 20 percent of their bone density — driven by the steep fall in oestrogen, which normally acts as a brake on osteoclast activity. What makes osteoporosis so insidious is that this entire process is completely silent. There is no warning signal, no alarm. The first sign that something has gone wrong is often a fracture — from a fall that, in a younger person with healthy bones, would have caused nothing more than a bruise. A hip fracture in an older woman is not just painful — it is associated with a significant increase in mortality risk within the first year.

Risk Factors: Are You at Higher Risk Than You Realise?

Some women are at significantly higher risk of osteoporosis than others. The main risk factors include:

  • Early menopause or premature ovarian insufficiency: The earlier oestrogen falls, the longer the bones are exposed to accelerated breakdown. Women who experience menopause before 45 should be assessed and considered for bone-protective treatment earlier.

  • Family history: If your mother or a close female relative had osteoporosis or a fragility fracture — particularly a hip fracture — your risk is meaningfully elevated.

  • Low body weight or a history of restrictive eating: Low body weight is consistently associated with lower bone density, and eating disorders are among the most powerful risk factors for osteoporosis in young women.

  • Long-term corticosteroid use: Steroid medications such as prednisolone, commonly used for asthma and autoimmune conditions, are one of the most significant pharmacological causes of bone loss.

  • Sedentary lifestyle: Bones strengthen in response to mechanical load and impact. Women who are physically inactive miss this stimulus and lose bone density more rapidly.

  • Vitamin D and calcium deficiency: Near-universal in urban India and directly correlated with bone density loss.

  • Conditions affecting absorption: Coeliac disease, inflammatory bowel disease, chronic kidney or liver disease, and hyperthyroidism all affect calcium and bone metabolism significantly.

How Is Bone Health Assessed? The DEXA Scan Explained

The gold standard for measuring bone density is a DEXA scan — Dual Energy X-ray Absorptiometry. It is quick, painless, and low-radiation, measuring bone mineral density at the hip and spine — the two sites most vulnerable to osteoporotic fractures. It delivers a result called a T-score: minus 1.0 or above is normal; between minus 1.0 and minus 2.5 is osteopenia (low bone density, a clear warning sign); minus 2.5 or below is osteoporosis.

At my clinic in Gurgaon, I recommend a baseline DEXA scan for all women over 50, for women over 40 with any risk factors listed above, and for any woman who has experienced an unexpected fracture. Blood tests including Vitamin D, calcium, parathyroid hormone, and bone turnover markers are a valuable complement to DEXA scanning in assessing bone health comprehensively.

Building and Protecting Your Bones: The Lifestyle Fundamentals

Now for the good news — and there really is a lot of it. Bone health is one of the areas of women's health most responsive to lifestyle intervention. Whether you are 28 or 58, the choices you make now have a measurable impact on your bone density.

Calcium — Your Bone's Primary Building Block

Women aged 19 to 50 need approximately 1,000 milligrams of calcium per day; women over 50 need 1,200 milligrams. The best sources in the Indian diet include milk, curd, paneer, sesame seeds (til) — one of the richest plant calcium sources available — ragi (finger millet), green leafy vegetables such as methi and drumstick leaves, and calcium-set tofu. If dietary intake is consistently insufficient — which it is for the majority of Indian women — a calcium supplement is appropriate.

Vitamin D — Without It, Calcium Cannot Work

Without adequate Vitamin D, your body can only absorb approximately 10 to 15 percent of the calcium you consume — compared to 30 to 40 percent with optimal levels. The recommended daily intake is 600 to 800 IU, rising to 1,000 to 2,000 IU for women over 50 or those who are deficient. I check Vitamin D levels routinely in all my patients over 35 and supplement accordingly. Sensible sun exposure — 10 to 20 minutes of midday sun on the arms and legs, without sunscreen, several times per week — is the most natural way to maintain Vitamin D, though supplements are necessary for most urban women.

Exercise — The Most Powerful Bone-Building Tool Available

Bone responds to mechanical loading by becoming denser and stronger. The most bone-protective forms of exercise are weight-bearing and impact activities — walking, jogging, dancing, stair climbing, yoga, and aerobics. Resistance training is particularly valuable because it not only loads bones directly but builds muscle mass that protects them in the event of a fall. I recommend a combination of 150 minutes of moderate weight-bearing exercise per week and two to three resistance training sessions for all my patients over 40. Swimming and cycling, while excellent for cardiovascular and joint health, are not weight-bearing and provide less direct bone benefit.

Protein — The Overlooked Bone Nutrient

Approximately 50 percent of bone volume is protein — primarily collagen — and adequate dietary protein is essential for maintaining bone matrix integrity. Many Indian women, particularly vegetarians, consume insufficient protein. The recommended intake is approximately 0.8 to 1.2 grams per kilogram of body weight per day. Dals, legumes, paneer, eggs, fish, chicken, soy products, and nuts are all excellent sources.

Medical Treatment for Osteoporosis — When Lifestyle Is Not Enough

For women who have already developed osteoporosis or significant osteopenia, lifestyle measures alone are usually insufficient and medical treatment is required alongside them. The options I discuss with my patients in Gurgaon include:

  • Hormone Replacement Therapy (HRT): For perimenopausal and early postmenopausal women, HRT is one of the most effective interventions for preserving bone density — directly addressing the oestrogen deficiency driving postmenopausal bone loss, while simultaneously relieving menopausal symptoms.

  • Bisphosphonates: Medications such as alendronate and risedronate slow bone breakdown and have strong evidence for reducing fracture risk. Typically taken weekly or monthly.

  • Denosumab: A six-monthly injection that inhibits osteoclast activity, used for women with more significant osteoporosis or those who cannot take bisphosphonates.

  • Calcium and Vitamin D supplementation: Always the foundation of any bone health treatment plan, regardless of which additional medication is used.

Fall Prevention — Because Protecting Bones Also Means Protecting Against Injury

No conversation about osteoporosis prevention is complete without talking about falls. A fragility fracture requires both a weak bone and an event that stresses it — for older women, that event is almost always a fall. Fall prevention strategies are a genuinely important part of comprehensive bone health management: adequate lighting in the home, removing trip hazards such as loose rugs, wearing supportive non-slip footwear, having regular eye checks, reviewing medications that cause dizziness, and maintaining good muscle strength and balance through exercise are all evidence-based measures that reduce fracture risk independently of bone density.

Start Now — Whatever Age You Are

I want to end with something I say to every patient who comes to me worried about osteoporosis: it is never too early and it is never too late. If you are in your 20s or 30s, the choices you make now are building the peak bone mass that will protect you for the rest of your life. If you are in your 40s approaching perimenopause, now is the time for a bone health assessment and a serious conversation with your gynaecologist. If you are already postmenopausal — come in. A DEXA scan gives us the picture we need, and there are excellent options available to stabilise and protect your bones at every stage.
