Women's Health After 40: The Complete Checklist Every Woman Needs — By Dr. Chetna Jain, Gynaecologist Gurgaon

Turning 40 is a milestone — professionally, personally, and medically. For women, the fourth decade marks a significant physiological transition. Hormones begin to shift, the risk profile for several chronic conditions rises, and the body starts sending signals that deserve careful attention. Yet in the busyness of careers, families, and daily responsibilities, women in their 40s are often the last to prioritise their own health.

I am Dr. Chetna Jain, a senior gynaecologist practising in Gurgaon, and I see this pattern every week. Women who have been putting off check-ups for years finally arrive at my clinic — sometimes prompted by a symptom that could have been caught much earlier. The good news is that with the right preventive approach, the years after 40 can be among the healthiest and most vibrant of a woman's life. This checklist is your roadmap.

1. Gynaecological Screening — Your Annual Non-Negotiables

Pap Smear and HPV Testing

Women aged 40 and above should continue regular cervical screening. A Pap smear every 3 years, or co-testing with an HPV DNA test every 5 years, remains the standard of care. Cervical cancer risk does not disappear after 40 — in fact, delayed or missed screenings in earlier years mean many women in their 40s are presenting with pre-cancerous changes that could easily have been detected sooner. Please do not skip this test.

Pelvic Examination and Transvaginal Ultrasound

An annual pelvic examination helps assess the uterus, ovaries, and surrounding structures for fibroids, ovarian cysts, or other abnormalities. A transvaginal ultrasound is recommended for any woman with irregular bleeding, pelvic pain, or a family history of ovarian or uterine cancer. After 40, the uterine lining (endometrium) should also be assessed if periods become irregular or unusually heavy — endometrial pathology becomes more common in the perimenopausal years.

2. Breast Health — Start Mammograms Now If You Have Not Already

Breast cancer risk increases with age, and the 40s are when regular mammographic screening becomes most important. The Indian guidelines for average-risk women recommend annual clinical breast examination and mammography every 1 to 2 years starting from age 40. Women with a first-degree relative (mother, sister) with breast cancer, or a personal history of BRCA mutation, should begin screening earlier and more frequently — this is something to discuss individually at your clinic.

In addition to mammography, I encourage all my patients to perform monthly breast self-examinations — ideally on the same day each month after their period. Familiarity with your own breast tissue is the most powerful tool for early detection. Any new lump, skin change, nipple discharge, or breast asymmetry should be reported to your doctor promptly.

3. Hormonal Health — Understanding What Is Changing and Why

The 40s are the decade of perimenopause for most Indian women. Oestrogen and progesterone levels begin to fluctuate unpredictably, and the symptoms — irregular periods, hot flashes, sleep disturbances, mood changes, brain fog, and weight gain — can begin a full decade before menopause itself. A hormonal assessment at 40 includes FSH, LH, oestradiol, AMH (Anti-Mullerian Hormone for ovarian reserve), prolactin, and a full thyroid panel (TSH, Free T3, Free T4, anti-TPO antibodies).

Thyroid disease — particularly hypothyroidism — affects one in eight to ten Indian women and becomes significantly more common after 40. Its symptoms closely mimic those of perimenopause, making it easy to miss without proper testing. If you have not had your thyroid checked recently, please make it a priority.

4. Bone Health — The Silent Risk That Starts Earlier Than You Think

Bone density begins to decline in earnest as oestrogen levels fall during perimenopause — which means the process of bone loss starts well before menopause itself. Osteoporosis affects a significant proportion of Indian women over 50, and many are already in negative bone balance in their 40s without knowing it.

A DEXA (Dual Energy X-ray Absorptiometry) scan measures bone mineral density and is recommended for women over 40 with risk factors — including a family history of osteoporosis, low body weight, a history of fractures, prolonged corticosteroid use, or early menopause. Vitamin D deficiency is near-universal in urban India and significantly accelerates bone loss. I check Vitamin D levels routinely in all my patients over 40 and supplement where needed. A calcium-rich diet, regular weight-bearing exercise, and adequate Vitamin D and K2 are the foundation of bone protection at this age.

5. Cardiovascular Health — Women's Heart Risk Rises Sharply After 40

Heart disease is the leading cause of death in women globally — and yet it remains dramatically under-recognised and under-investigated in women, who often present with atypical symptoms that differ from the classic male presentation. Oestrogen has a cardioprotective effect, and as levels begin to fall in the 40s, cardiovascular risk rises. The following checks are essential after 40:

  • Blood pressure measurement — at least annually; hypertension is the single biggest modifiable cardiovascular risk factor

  • Fasting lipid profile (total cholesterol, LDL, HDL, triglycerides) — annually or every 2 years depending on results and risk factors

  • Fasting blood glucose and HbA1c — to screen for prediabetes and Type 2 diabetes, both of which increase cardiac risk substantially

  • Body weight and waist circumference — central (abdominal) obesity is a stronger cardiac risk predictor in women than BMI alone

6. Mental Health — The Most Overlooked Aspect of Women's Health After 40

The hormonal turbulence of perimenopause has a direct neurological impact — affecting serotonin and dopamine levels in ways that increase vulnerability to anxiety, depression, and mood disorders. In India, where mental health remains heavily stigmatised, many women in their 40s are quietly suffering from clinical anxiety or depression while continuing to manage families and careers without any support.

I routinely screen my patients over 40 for mood disturbances using validated tools — because a woman who ticks every physical health box but is struggling emotionally is not truly well. 7. Eyes, Teeth, and Skin — Health Areas Women Often Defer

After 40, eye examinations become more important — glaucoma, macular degeneration, and presbyopia all become more common. An annual eye check is recommended. Dental health is directly linked to cardiovascular health — gum disease is an independent risk factor for heart disease and diabetes. Six-monthly dental check-ups should be a standard part of your health calendar. Skin changes — including new moles, pigmentation, or growths — should be evaluated by a dermatologist annually.

  • Pap smear and HPV test — every 3 to 5 years

  • Pelvic examination and ultrasound — annually

  • Mammogram — every 1 to 2 years from age 40

  • Full hormonal panel including thyroid — annually

  • DEXA bone density scan — as recommended based on risk factors

  • Blood pressure, lipid profile, fasting glucose, HbA1c — annually

  • Mental health screening — at every gynaecological visit

  • Eye and dental examination — annually

  • Skin check — annually
